Medical Orders Scope of Treatment
“Medical Orders for Scope of Treatment,” known as M.O.S.T., are orders that direct your care in a medical crisis. These orders indicate what treatments you want now in your current state of health.
M.O.S.T. are typically used for patients who have advanced illnesses and may pass away within the next year. Completing this form is completely voluntary. Because the form is a set of medical orders, it requires a physician’s signature.
“Spanish MOST document is to be used for conversational assistance with Spanish speaking patients. Only the English version should actually be signed by the patient and forwarded for physician signature as caregivers need their instructions/orders in English”
